All One Life Health Group staff will endeavor to try and collect all your personal information directly from you, yourself. This may take place when you complete admission or administrative paperwork on arrival or via secure online forms. It may also occur through Practitioners in their treatment rooms, during your treatment or over the telephone. We may also collect your personal information when you visit our website, send us an email or SMS, or make an online appointment or communicate with us over social media.

- Information that we consider necessary to assist our practitioners in your diagnosis, treatment and management of your health condition.
If you do not wish for certain information to be collected, you will need to discuss this with your practitioner, so they can disclose any consequences this may have for your care.

Storing of your personal information is done purely in an electronic format. The security of your information is important to One Life Health Group and is taken very seriously. Our staff are responsible for maintaining the security of patient information from unauthorised access, loss or damage.
